Setting up is done as you install a usual module. You can install modules only once, but use as much instances of it as you like.
Go to system / install extensions, and upload the module there. Now it is installed and you can publish the module in an article, or module position.
Once done, you can publish the module and its contents. The table view you will see will have the first columns be the core joomla columns, and next to them you will find the custom user field columns you have selected.
